2015

  • NCHA named a SCALE partner community by the Institute for Healthcare Improvement.
  • Make TODAY Count! Community Care Corps Summer Lunch Program provided 1160 free meals for people from the community surrounding the NCHA main offices.

2014

  • My Health Connections opens as a walk-in assistance center to house the Health Coverage Guides and act as a community wellness and information space.
  • Thriving Weld Data Dashboard goes live.
  • Make TODAY Count! along with 30 other partners organized the Weld County Summer Initiative and engaged over 3,000 local children in learning about how to make healthier choices in their drinks and how to be more active.
  • Launched the Community Murals program with five murals currently painted to-date and more in the works.

2013

  • Make TODAY Count! Campaign launched to implement Weld County Community Health Improvement Plan with support from Kaiser Permanente.
  • Connect for Health Colorado contract awarded to NCHA to support local, mobile Health Coverage Guides to help enroll the local community in affordable health insurance.
  • A Colorado Adult Medicaid Quality Measure (CAMQM), a depression screening program, designed and implemented in 30 days.
  • Community Care Corps organized and deployed to build relationships with seven at-risk communities.

2012

  • North Colorado Health Alliance contracted with Colorado Access to provide care management for Regional Care Collaborative Organization Region 2.
  • North Colorado Health Alliance contracted with Rocky Mountain Health Plans to provide care management for Regional Care Collaborative Organization Region 1 (south Larimer county).

2011

  • Sunrise Community Health opens embedded primary care clinic at North Range Behavioral Health.
  • Sunrise Community Health opens embedded primary care clinic at Larimer County Mental Health (now Touchstone).
  • The Colorado Health Foundation funds Healthy Eating, Active Living at Weld County Department of Public Health and Environment.

2010

  • The Colorado Health Foundation funds embedded Sunrise Community Health clinic at Larimer Center for Mental Health.
  • The Colorado Health Foundation funds North Colorado Health Alliance infrastructure.
  • Banner Health supports primary care expansion at Sunrise’s Monfort Family Clinic.
  • North Colorado Health Alliance initiates regional discussions re: Health Information Exchange (HIE), transitions of care, integration, service expansion.
  • North Colorado Health Alliance partners with Colorado Access to submit RCCO Region I application.
  • Sunrise’s Kids Care Clinic expands.
  • Sunrise Community Health initiates high school job shadowing and internship program with University Schools & Jefferson High School.

2009

  • Caring for Colorado funds pediatric oral health expansion at Sunrise Community Health.
  • Private Physician Volunteer Corp expands to build primary care capacity and specialty service – initiated by private physicians wanting to build on existing safety net infrastructure.
  • The Colorado Trust funds expansion of children’s health services in southern Weld County.
  • The Colorado Health Foundation funds the North Colorado Health Alliance for Integrated Behavioral Health HIT.
  • The Colorado Health Foundation funds Sunrise Community Health for Strengthening the Safety Net.

2008

  • Colorado Department of Public Health and Environment funds for Cultural Competency Training and Development.
  • The Colorado Health Foundation funds the North Colorado Health Alliance for Integrated Behavioral Health Service expansion.
  • Island Grove Regional Treatment Center and North Range Behavioral Health merge.
  • Sunrise Community Health expands case management to include prenatal, pediatric, oral health, cardiovascular, hypertension, pain management, tobacco usage, TB, migrant, and diabetic patients.

2007

  • North Range Behavioral Health fully integrates outpatient Behavioral Health at Sunrise Community HealthSunrise Community Health’s Monfort Family Clinic opens, withsupport from Banner Health, Monfort Family Foundation, NCMC Inc, NCMC Foundation, City of Evans, The Colorado Health Foundation, Caring for Colorado, and federal funding.
  • The Colorado Health Foundation funds the North Colorado Health Alliance for HIT Partnership.
  • The North Colorado Health Alliance participates in Institute for Health Care Improvement’s Triple Aim pilot.

2006

  • The North Colorado Health Alliance initiates second Weld County comprehensive community Needs Assessment & Health Care Assets Inventory.
  • The North Colorado Health Alliance completes Health District Feasibility Study.
  • The Colorado Health Foundation funds the North Colorado Health Alliance for Integrated Behavioral Health Center of Excellence.
  • Sunrise Community Health opens Kid’s Care Clinic.

2005

  • The North Colorado Health Alliance Initiates new Specialty Clinics at Sunrise Community Health: Ophthalmology, OB/GYN, Podiatry, ENT, Asthma & Allergy, ADHD.
  • The North Colorado Health Alliance nitiates Primary Care Capacity Task Force.
  • The Colorado Trust funds Behavioral Health Latino Professional Development Program at North Range Behavioral Health.

2004

  • The North Colorado Health Alliance Implements an integrated Electronic Health Record at Sunrise Community Health and the Weld County Department of Public Health and Environment.
  • Sunrise Community Health opens school-based health centers at two elementary schools in Greeley/Evans.

2003

  • NCMC Inc, Caring for Colorado, and Weld County Department of Public Health and Environment fund the North Colorado Health Alliance’s new mobile medical van.
  • The North Colorado Health Alliance initiates system-wide cultural and linguistic competency program.
