An interactive, public-facing community health data dashboard that includes strategic planning and performance measurement applications
Thriving Colorado Dashboard
The statewide equivalent of our Thriving Weld Dashboard. This project brings together organizations from across the state for the purpose of achieving collective impact.
Healthy Schools
As part of the Healthy Schools initiative, NCHA provides resources, support, and expertise for planning and implementing wellness initiatives and wellness polices in local Northern Colorado school districts.
My Health Connections
My Health Connections serves as a dedicated assistance site and has appointment and walk-in availability. Trained professionals, known as Health Coverage Guides (HCGs), assist Weld County residents through the complexities of purchasing health insurance online.
Regional Accountable Entity (RAE)
Regional Health Connector
The North Colorado Health Alliance hosts the Regional Health Connector (RHC) for Weld County. RHCs are local residents whose full-time job is to improve the coordination of local services to advance health and address the social determinants of health. They focus on connections among clinical care, community organizations, public health, human services and other partners.
Making an Impact Across Northern Colorado
The North Colorado Health Alliance convenes partners and co-workers in education, community service, health care, business, faith-based organizations, and government. The Alliance’s mission is to see Northern Colorado become the healthiest region in the healthiest state by developing and supporting a healthy population with 100% access to quality service and care at a sustainable cost. The Alliance works through collaborative efforts that bring multiple sectors together to address the health of the local community. The Alliance recognizes that improving community health requires multiple partners and multiple sectors working in tandem toward a collective goal. Currently NCHA employs 72 people through 11 different grant programs spread across three counties and three office locations.
