Since 2013, North Colorado Health Alliance has served as a Certified Assistance Site and Enrollment Center for Weld County, providing impartial assistance to consumers seeking health coverage through the health insurance marketplace, Connect for Health Colorado.

In partnership with Connect 4 Health Colorado, NCHA’s My Health Connections team of Certified Health Coverage guides work with Northern Colorado residents to navigate the complexities of purchasing and maintaining Health Insurance coverage.
All Health Coverage Guide services are provided free of charge.

Anyone experiencing a Qualified Life Change Event or difficulties in maintaining coverage can also visit us for assistance.
1. Health Insurance can not be denied due to a pre-existing condition
2. Individuals and families who purchase coverage through Connect 4 Health Colorado may be eligible for financial assistance to reduce their monthly premium cost
3. A Qualified Life Change Event (loss of health insurance coverage, change in income, marriage, birth or adoption of a child, divorce, and relocation to a new zip code) presents opportunities for those seeking health insurance coverage to enroll in the C4HCO marketplace at any time during the year
*Open enrollment is November 1 – January 15
4. In Colorado, families as well as adults who do not have children, may be eligible for Medicaid based on income (and can apply at any time during the year

Connect for Health Colorado® is Colorado’s official health insurance marketplace, and the only place to apply for financial help to reduce your monthly health insurance costs. The financial help you can get through Connect for Health Colorado to lower your monthly payment is called a Premium Tax Credit. Coloradans that qualify for financial help may also qualify for healthcare discounts, called cost-sharing reductions, that lower the amount you pay when you use your plan. Healthcare discounts are only available on silver plans sold through Connect for Health Colorado.
My Health Connections is a Connect for Health Colorado Enrollment Center in Evans/Greeley with certified assisters to help you apply for financial help, help you understand the plan options, and help you sign up for the plan that you find best meets your needs. All health insurance plans sold through Connect for Health Colorado cover essential health benefits and you cannot be charged more or prevented from getting the care you need because of a pre-existing condition.
